The Transition Program at Polk State College is a partnership between the University of South Florida and the School District of Polk County. Students who have earned a special diploma enhance their self-determination, employability and independent living skills by working various jobs on and off campus, accessing public transportation, interacting with same-aged peers, and participating in a curriculum geared towards their individual transition needs. The goal is to create a seamless transition from school to adult life.
